SystmOnline can now offer you a summary view of your medical records online. If you wish to sign up to this you will need to come into the surgery and fill in a form (proof of identity is required). If you send a representative we will need your written consent. A username and password will be issued to you and this will give you access to your repeat medication list, the ability to book appointments and a summary of your medical records.

A username and password will be issued to you and this will give you access to your repeat medication list, the ability to book appointments and a summary of your medical records. There will be no requirement for Surgeries to complete a Survey this year as this is being replaced by the 'Friends & Family Test'.
The question in the 'Friends & Family Test' is: How likely are you to recommend our services to friends and family if they needed similar care or treatment?' These answers generate scores and once these are determined, the result is sent to NHS England on a monthly basis to be published on the NHS Choices website. Please look out for the papers containing these questions which will be in Reception from December 2014.
