This information will only be used to review treatments and progress future treatments, it will be stored for 7 years as is the required time for insurance purposes. Customers reserve the right to have these records deleted at anytime. Personal information will not be shared with any third parties. We will only use contact information to contact customers directly concerning future and past treatments.
